Overview

Train and consult with pregnant women to help them achieve adequate prenatal and post-natal care as well as encourage proper care of their infants through age 2 through a home visiting approach.

Responsibilities

Develop knowledge competencies regarding evidence-based best practices for maternal child health and home visiting as directed by supervisor.

Visits in home and community settings with pregnant and post-partum women and/or fathers regularly from the first confirmation of pregnancy until the infant's second birthday, utilizing approved curriculums and evidence-based best practices.

Assists with families keeping medical appointments, including coordination of transportation.

Assists with the developing plans for baby's care - including medical, social, emotional and child care.

Assists with clients staying in school and/or continuing to work.

Educates on family planning to avoid repeat pregnancies and achieve pregnancy spacing.

Assist in day-to-day problems involved with being a new parent.

Participates in committees and coalitions, as directed.

Assists in creating program materials, as needed Coordinates participation in support group and group classes.

Facilitates group educational sessions.

Finds new outreach sources and resources for client needs.

Helps clients to apply for FAMIS insurance and/or access affordable health care through local free/reduced cost clinics.

Assists in training new employees.

Provides input to and participates in quality assurance activities to increase client participation and improve client outcomes.

Participate in staff meetings and one on one supervision with supervisor.

Adhere to all guidelines and protocols required by the program.

Performs other duties as assigned.

Qualifications

High school plus three (3) years related community experience working with families and children. Experience in a home visiting or case management program preferred. Understanding of health insurance (emphasis on FAMIS and Medicaid programs) and local support agencies and resources (such as Dept. of Social Services and local federally funded community health centers). Basic computer skills in Microsoft Office and a valid driver's license with safe/reliable transportation required. Bi-lingual (Spanish) a plus.

Valid Driver's license, provide a recent DMV report and have use of a vehicle in good working order with proof of insurance with a minimum limit of liability of $100,000 per accident as use of a personal vehicle will be required as part of this position. Copies of all documents must be provided to Human Resources, prior to any use of a personal vehicle for this position.
