Sentara Medical Group is looking for an innovative, compassionate APP to join our growing palliative care program. This APP role will develop and support the building of a new home-based palliative care program. This individual will be part of an interdisciplinary team consisting of RNs and an MSW who will collaboratively provide palliative care services. This role will include both home and telehealth based palliative care visits.Qualifications:
